This website is designed to collect only the information used to review a visit request. After your final confirmation, the website transmits the form to AthenaCare's server, creates a PDF intake form, and delivers it through AthenaCare's configured intake workflow.

Information you choose to provide
The Request a Visit form is transmitted to AthenaCare's web host and approved receiving service. The website application does not place the completed form in a database or permanently save the generated PDF; the PDF is generated in server memory and attached directly for delivery without password protection. Hosting and delivery systems may still process or retain the submission, transmission metadata, and messages under their own configurations and policies.
Website information
Basic technical information, such as an IP address, request time, browser information, and security events, may be processed by the hosting service to deliver the site, limit abuse, maintain security, and understand reliability. When the Request a Visit form is submitted, the site creates a strictly necessary, session-only security cookie to protect against cross-site requests. It is not used for advertising or tracking and expires with the browser session. The application is designed not to write patient form contents into its own error or rate-limit logs.
Medical and health information
The Request a Visit form requires a Medicare Beneficiary Identifier (MBI) in its designated Medicare Number field because AthenaCare currently serves Medicare patients. The MBI is used to review the request and is included in the intake PDF delivered through AthenaCare's configured intake workflow. Do not place Social Security numbers, detailed medical histories, records, other health-plan identifiers, or urgent symptoms in open-text fields or standard email. Technical safeguards do not replace AthenaCare's legal, privacy, security, access-control, vendor, and records-management obligations.
