Glendale, California office Call (747) 236‑0010 Care type and availability are confirmed before scheduling.

How it works

A straightforward path to the right next step.

The process is designed to gather only the basic information needed to begin, then confirm details directly with the care team.

01

Request care

Complete the Request a Visit form, including the required Medicare Beneficiary Identifier (MBI), review the information, and submit it directly to AthenaCare through the website. Nothing is sent until you choose Submit Visit Request. You may also call AthenaCare directly.

02

Speak with our team

A team member reviews the request, asks any necessary questions, and explains what information is needed next.

03

Confirm the care option

AthenaCare confirms Medicare information and whether the requested visit type is appropriate, currently offered, and available in the patient’s location.

04

Plan the next step

If the request can move forward, the team explains scheduling, preparation, and what to expect.

Prepare for the call

Keep the first request simple.

Do not send detailed medical records, Social Security numbers, Medicare Numbers (MBIs), or other health-plan identifiers through standard email. Enter the MBI only in the designated secure Request a Visit field.
